Liberty Doctors was established 10 years ago by seven primary care providers with a commitment to providing patient-focused care. With 53 providers serving patients in 16 practice locations, the focus remains the same: care with a personal touch.

Personalized care goes hand-in-hand with an enhanced patient experience. This involves leveraging advanced tools to support care delivery, including state-of-the-art electronic medical records, a user-friendly virtual telehealth platform, text message reminders and patient satisfaction surveys. Liberty Doctors also has implemented care management programs that actively engage with patients beyond their primary care visits, focusing on individualized metrics and trends to help improve overall health. In addition, Liberty Doctors has a practice led by John Forney, M.D., that brings care directly to homebound patients.
“Not only do we evaluate test results and measurements from these diagnostic tools, we consider the whole person as we determine the best mode of care for our patients,” Dr. Kinney explained. “Are they facing anything at home that could negatively impact their hypertension or diabetes? Do they have financial stability or are they purchasing less expensive items at the grocery store or having to choose between groceries or paying the mortgage or rent?”
