At Agape Hospice we provide excellent care for patients and families going through one of toughest times of their lives. Agape Hospice services are available to patients who no longer benefit from, or choose to forgo, curative treatment. The typical hospice patient has a life expectancy of six months or less, if the illness takes its expected course. Most patients receive care at home or in a long-term care setting such as a nursing home or assisted living community. Services are provided by a team of trained professionals who provide medical care and support services, not only to the patient, but to the patient’s family and caregivers as well. Together with the patient’s attending physician, the hospice care team establishes and monitors a Plan of Care that incorporates reasonable and necessary medical and support services for the management of the terminal illness. Part of this plan is to avoid unnecessary hospital stays and emergency room visits. We love our patients and families and give them the best care possible.
