Apart from serving 20 years in the Navy as a credentialed emergency room trauma support doctor with two deployments to Afghanistan, Charleston native William Melvin Brown III, MD, has spent his career as a board-certified emergency room specialist.
During the COVID pandemic, however, he reached a breaking point, frustrated about how values-driven medicine had seemingly evaporated. “As more patients equals more money, the big profits are derived from quota and quick turnover,” he explained. “For example, if each doctor sees 2,000 to 3,000 patients a year, that breaks down to approximately 25 per day. The system is set up for failure because reacting to illness is not proactive.”
A New Approach to Patient Care
By 2022, Dr. Brown decided the time had come for a change. Wanting to make a difference as an advocate for meaningful, lasting doctor-patient relationships, he crystallized his vision over the two years he worked with a private practice on James Island. Envisioning a way to offer a refreshing, human-centered approach that restores trust in health care and personal connection in medicine, he launched his own concierge practice, Charleston MD, in 2024.
Prioritizing Communication and Time
Dr. Brown’s compassionate medical practice embraces the concept of respectful communication between himself, his staff and his patients. “Managing 350 patients a year rather than 2,000 is a totally different mind-set,” he said. “Using this model of health care the way it used to be, I’m able to foster patient empowerment when it comes to medicine as well as shared decision-making.

Another benefit of Dr. Brown’s practice is that patients are able to speak directly with him without having to navigate layers of staff or bureaucracy. He is on-call nights and weekends and doesn’t bother with an answering service.
“Charleston MD is changing the narrative of modern medicine from patient turnover to prioritizing customer service,” he said. “Our team, including nurse Janice Rama and front desk manager Heidi Hodges, have transformed a regular doctor’s office into a spa-like, calming environment with only four chairs in the waiting room.
In the year since he started his practice, building from zero patients, Dr. Brown said, “I’ve found where I need to be in my life. I’m happier than I’ve ever been.”
